NEW 2020-2021!!! Athletic Physical Update

If a student had a valid physical for the school year 2019-2020, he/she will not be required to get another physical done for the 2020-2021 school year.  If the student chooses not to get a physical for the 2020-2021 school he/she will need to complete this sports  Health Questionnaire form by a parent/guardian (this is a 2 page form), then turned into the athletic office.

 The middle school office is now open.  Our hours for assisting families are:  7:30-3:00.  If you plan to visit us in the building please be sure to wear a mask and log yourself in with a temperature check upon entering.
